He was predeceased by: his parents, James Fillmore Cook and Velma Gertrude Hammock Cook; his wife Jamie Dawn Taylor Cook; his daughter Leah Cook; and his brothers, Phillip Cook and James Michael Cook (Joyce).
He is survived by: his son Jarrod Cook (Alexandra Lenox); his grandson Cypress Cameron Cook; his close friend Teresa Dye; and his brother Larry Cook (Rita). He is also survived by several nieces, nephews, extended family, and friends.
