He was predeceased by: his parents, William Hawkins and Wilma Stafford Hawkins; his wife Carol Janet Perdue Hawkins; his granddaughters, Sherry and Shannon Hawkins; and his daughter-in-law Sherry G. Hawkins.

He is survived by: his children, David Hawkins Jr., Chris Hawkins (Christie), Maridee Kivett (Bobby) and Michelle Phelps; his grandchildren, Stephanie Young (Scott), Shaun Hawkins, Shane Hawkins, Aaron Hawkins, Abigail Canipe (Daniel), Justin Kivett (Sarah), Travis Kivett (Shelly), Courtney Caudill (Joel), Lauren Hayden (Dan), Brittany Etheridge (Jared), Andrew Phelps, Kelsey Phelps and Sydney Phelps; his great grandchildren, Paige Hawkins, Marshall Hawkins, Stetson Hawkins, Madelyn Kivett, Rebekah Kivett, Aubrianna Kivett, Emma Kivett, Alexa Kivett, Addison Kivett, John Kivett, Isabella Kivett, Ezra Etheridge, Jude Etheridge, Simeon Hayden and Caudill; and his siblings, Barbara Beal (Don), Carolyn McClure, Dianne Roudabush (David), Joel Hawkins (Sherry) and Stephen Hawkins.
