She was predeceased by: her parents, Parley Pratt Formyduval, Sr. and Lois Ludlum Formyduval; her husband James Thurman Simmons (J. T.); and her brother Praley Pratt Formyduval, Jr.. She is survived by: her sons, Davis Simmons (Aimee) of Waxhaw, NC and Jamie Simmons of Ash, NC; her sister Avis Williams of Greer, SC; and her grandchildren, Greyson and James Simmons.

In lieu of flowers the family has suggested memorials be made to Soldier Bay Baptist Church, PO Box 10, Ash, NC 28420.
