He is survived by: his parent Kate Hazlett (McAllister); and his children, James A. Hazlett of Coupeville, WA, Walter I. Hazlett of Ashburn, VA, Susan G. Hazlett of Arden, NC, Douglas B. Hazlett of Covington, KY and William L. Hazlett of Phoenix, AZ.
