He was predeceased by: his parents, George Wayne Polk and Ruby Ray Polk. He is survived by: his fiancée Wanda Gail Locklear of Albemarle; his daughters, Hailey Smith (Ben) of Albemarle and Bridget Kaylor; his sisters, Vivian Gregory (Buddy) of Norwood and Crystal Wooten (Mike) of Dallas, NC; and his grandchildren, Cadie, Caitlynn, Camden, Cole, Nyoka and Winter.
