He was predeceased by: his parents, Enoch Cardwell Cook and Rosa Belle Royal Cook; his wife Johnsie Kay McGee Cook; and his brothers, John Cook and Gary Cook.
He is survived by: his children, Daryl Cook (Gwen) and Kim Miller (David); his grandchildren, Ben Miller (Liz), Andrew Miller (Holly), Alex Miller, Maggie Miller, Mechim Cook (Liana) and Grayson Cook; his great grandchildren, Charlotte Miller, Pax Miller, Lili Miller and Liam Miller; and his sisters, Betty Jean Tedder and Brenda Faye Larimore.
