He was predeceased by: his parents, Julius Kozak and Bernice Kozak (Qualley). He is survived by: his siblings, Ron and Betty Jean; his wife Caroline Hansen; his granddaughter Karli; his daughters, Judy Kozak and Rhonda Parks (Eddie); his siblings, Ronald Kozak and Betty Johnson (Don); and his brother-in-law Stanley (Nancy). He is also survived by seven nieces; two nephews, and numerous great-nieces and nephews.
