He is survived by: his parents, Matthew Yakubowski and Jessica Yakubowski; his grandparents, Vincent and Lisa Yakubowski; his uncle Timothy Yakubowski; his grandparents, Bradley and Donna Perkins; his aunt Nicole Burris (Ashley); his cousin Braxton; his aunt Amanda Marcellais; and his cousin Zariah.
