She was predeceased by: her parents, Clarence McClain and Inez McClain; her husband Glen Forman; her brother Cecil McClain; her son Glen Forman Jr.; and her children-in-law, Peter Keller, Rosalinda Forman and Laurie Forman.
She is survived by: her children, Dewayne Riddell, Kim Riddell, Karen Waytausky, John Waytausky, Ronald Forman, Jim Lakes, Sherry Lakes, Pam Lakes, Kenneth Forman, Deborah Forman Keller, Benjamin Forman and Joelle Forman; and her brother William McClain. She is also survived by 39 grandchildren, 31 great-grandchildren, and two great-great-grandchildren; and many dear friends.
