He is survived by: his parents, Richard Cook and Marva Cook (Dana); his wife Donna Cook; his son Richard III Cook (Jennifer); his grandchildren, Wren and Charlotte; his children, Dalton Cook and Kiera Johnston (Dalton); his granddaughter Emily; his children, Kaitlyn Cook (Charles), Emmitt Cook and Ahnna Cook; his brother Chad Cook (Raychel); and his sisters-in-law, Cecilia Bjornestad (Adam) and Brandy Johnson (Quinn). He is also survived by Numerous nieces, nephews, extended family and many friends.
