He was predeceased by: his parents, Howard V. Moak and Josie N. Moak; and his brothers, Howard Wayne Moak, Johnny M. Moak and Ellis Ray Moak.

He is survived by: his wife Erma Moak; his daughters, Mary Ann Treadway and Jeanne Deniece Hebert (Larry); and his siblings, Richard Carroll Moak (Dolly), Jimmy G. Moak (Joy) and Rose Mary Pickering (Garry). He is also survived by other loving family members and friends.
