He was predeceased by: his parents, Mansfield Cook and Lou Ann Null Cook; his daughters, Amanda Wood and Rebecca Wood; his brothers, Thomas Cook, Bill Cook, Ted Cook and Melvin Frashier; his grandson William Wesley Beavers; his stepgrandson Robert Clinton Smith; and his stepgreat-grandson David Christian Smith.
He is survived by: his wife Judy G. Cook; his daughter Sandra Beavers; his stepdaughters, Melissa Copeland (Paul) and Teresa Richardson (Kenny); and his siblings, Donald Cook (Rita), Gene Cook (Julie), Fred Cook, Roger Cook and Joyce Clark. He is also survived by ten grandchildren, nineteen great grandchildren, and a host of nieces, nephews, cousins and friends.
