He was predeceased by: his parents, Leslie Gerald Derrick and Louise Conklin Derrick. He is survived by: his grandchildren, Peyton, Jackson Rainer, Kaylyn, Michael May, Ryan, John Reed Derrick and Cooper Jackson; his wife Wanda Derrick; and his children, Dana May (Daniel), Amy Rainer (Drew), Ian Derrick (Allie), Jason Jackson, Brent Jackson, Howie Starnes and David Derrick Jr..
