He is survived by: his wife Madie Holder Cazzell; his children, Johnny Cazzell, Dalton McMillan (Janice), Tammy Hebert (Dennis), Phillip McMillan (Cindy) and Donna Butinich (John); his daughter-in-law Renee Cazzell; his grandchildren, Tasha Guice (John), Dusty Young (Matt) and J. J. Cazzell; and his great grandchildren, Trip, Ada of Scarlet, J. T. and Gracie. He is also survived by numerous relatives, grandchildren, and great-grandchildren.
