He is survived by: his wife Mary Agnes Herrin Middleton; his daughters, Traci Middleton Cothern (Chris) and Angela Middleton Diedrick (Brian); and his grandchildren, Shan Michael Cothern, Addie Marie Cothern, Drake Alexander Diedrick and Drew Franklin Diedrick.
