He is survived by: his mother Kimberly York Cook; his stepfather David Cook; his father Rick Leverette; his stepmother Lisa Leverette; his siblings, Jared Leverette, Luke Leverette, Rick Leverette, II and Grace Leverette; his uncles, Mark York (Amy), Ted Leverette (Angela) and Patrick York (Jana Hargis); his aunt Jamie Moon; and his close friends, Austin York, Tara Lollar, Ken Schroeder, Kevin Forrester, Art Johnson, Matt Cook, John Allen Jones, Mike Boyle and Mike Thompson.
