He was predeceased by: his parents, Travis Polk and Peggy Jo Johnson. He is survived by: his significant other Angela Thorton of Morton, MS; his stepmother Charlotte Polk of Morton, MS; his daughter Christin Polk of Forest, MS; his siblings, Judy Northway of Vicksburg, MS, Randy Polk (Carmen) of Pelahatchie, MS and Kevin Polk of Forest, MS; and his stepbrother Gary Cushman of Forest, MS. He is also survived by several nieces and nephews.
Visitation was held on Thursday, September 26th 2024 from 5:00 PM to 8:00 PM at the Wolf Funeral Services - Morton (3054 US-80, Morton, MS 39117).
