He was predeceased by: his parents, O. G. Polk and Maggie Chance Polk; and his brothers, Oscar and Danny Polk. He is survived by: his wife Marcia Polk of Monroe, GA; his sons, Greg Polk of Stephens, AR, Brad Polk (Jennifer) of Loganville, GA and Adam Polk of Monroe, GA; his sister Linda McPhail (Glenn) of Prentiss; his grandchildren, Dalton Polk (Kaidy), Katelyn Polk, Jared Polk, Jaden Polk and Jackson Polk; and his great grandchild Raley Polk.
