He is survived by: his wife Judy Polk of Prentiss; his children, Rhonda Tolar (Todd) of Columbia and John Lowery Polk of Prentiss; his grandchildren, John Ashton Polk, Macayla Polk, Jana Ryals (Brian), Randa Carr (Stephen) and Holly Tolar; and his sisters, Shirley Stough (Jerry) of Picayune and Joann Montgomery (Whit) of Birmingham. He is also survived by 3 great-grandchildren.
