He was predeceased by: his parents, Johnny Lee and Effie Louise Tackitt Forman; his son Marty Forman; his brother Tommy Forman; and his brothers-in-law, Terry Rackley, Gary Graham and Norris Graham.
He is survived by: his wife Loyce Graham Forman; his children, Angela Tackett and Danny Forman; his siblings, Kay Whitehead (Gerry), Denise Rackley and Bobby Forman (Hazel); his sister-in-law Joyce Forman; and his grandchildren, Jeremy Graham, Devin Forman, Neal Tackett, Bradley Forman, Santana Tirado, Immanuel Forman and Joseph Forman. He is also survived by twelve great-grandchildren.
Memorials may be sent to the American Cancer Society or to St. Jude Children's Research Hospital.
