He is survived by: his wife Cindy Cook Conrad of Taylor, MS; his children, Virginia Conrad Hale (Brente) of Taylor, MS, Natalie Ann Smith (Jeremy) of Oxford, MS, Kenny Hazlewood (Kaytee) of Oxford, MS and Tommy Hazlewood (Caralee) of Oxford, MS; and his siblings, Retha Marie Mills of Mountville, SC and Chad Alan Conrad of Oxford, MS. He is also survived by eleven grandchildren.
