She was predeceased by: her father Herbert W. Whitehead. She is survived by: her mother Hazel Smith of New Albany; her daughter Destiny "Jade" Mooneyhan; her sisters, Marilyn, Tammy and Sandie; and her grandchildren, Jaden, Cameron and Alexis.
