She was predeceased by: her parents, Alvin Andrew Evans and Martha Lou Dunn Boutwell; and her siblings, Betty Lou McNeely, Robert Evans, Dorothy Jean Scott and Billy Ray Evans.
She is survived by: her children, Michael Lee Slater (Hollie) and Destiny Elizabeth Lewis; her grandchildren, Lilybeth Slater, Jason Cagle, Camryn Price and Claire Notter; and her siblings, Thomas Terrill Evans (Darlene), Gary Wayne Evans (Cyndi) and Marla Jo Akins. She is also survived by numerous nieces, nephews, other family and friends.
