He was predeceased by: his grandparents, Braxton and Ethel McKinley; his uncles, Jean A. Kuyrendall and Glen Fortenberry; and his sister-in-law Kelly Kuyrkendall.
He is survived by: his parents, Carl Kuyrkendall and Sue Kuyrkendall; his siblings, Debbie Butler and Lee Kuyrkendall; his brother-in-law Billy Butler; his nieces, Jennifer Butler, Stephanie Butler and Kasey Kuyrkendall; his great-niece Carlee Addison; and his aunts, Annette Fortenberry, Betty Kuyrkendall, Donteal and Ance Grubbs.
