She was predeceased by: her parents, Henry Pearl Dykes and Hettie Bullock Dykes; her siblings, Elsie Love of Doris Lenoir, H. P and Glen Dykes; and her daughters-in-law, Edy Forman and Alisa Forman.
She is survived by: her children, Kay Williams (Mike), Van Forman, Janet Welch (Harold) and John Forman; and her grandchildren, Scott Williams, Shelley Davis (John), Kristy Warren (Danny), Drew Welch Ben (Jeri), Elizabeth Forman Kapler and Kelly Forman (Courtney). She is also survived by great grandchildren, great-great grandchildren, and nieces and nephews.
