He was predeceased by: his parents, Ardry Van and Mamie Toney Forman; and his siblings, Emerson Forman, Noel Forman, Barbara Garner, Jerry Foreman, Sue Stockton, James Forman and Betty Sandifer.
He is survived by: his wife Deborah Coker Forman; his children, Jason Forman (Brett), Jarrett Forman (Ashley) and Bambi Cook (Chris); his sister Mamie Kirk (David); and his grandchildren, Mary Ella Cook, Brody Cook, Lane Forman and Charlotte Forman. He is also survived by numerous nieces, nephews, and beloved sisters-in-law and brothers-in-law.
