She was predeceased by : her parents, Ardry Van Forman and Mamie Toney Forman; her former life partner Sandy A. Simmons; her husband William; her daughters, Janet Simmons Reid, Sandra Faye Simmons and Jennifer Lee Simmons; her grandson Matthew Todd Simmons; her siblings, Sue Evelyn Forman Stockton, Emerson "Embo" Forman, Noel "Bear" Forman, Jerry "Fat Man" Forman and Jamie "Foozie" Forman; and her siblings-in-law, Betty Bryant Forman, Kay Prine Forman, Paula Llorens Forman, Aubrey Sandifer and Merle Stockton.
She is survived by : her son William Lloyd Simmons; her children-in-law, Ashley of Magnolia, MS and Morty Reid of Liberty, MS; her grandchildren, Casey Reid of Liberty, MS., Marci Reid of Liberty, MS. and Corey Reid of Liberty, MS.; her great grandchild Tyler Whittington Reid of Liberty, MS.; her siblings, Glenn Forman (Deborah) of Magnolia, MS, Betty Forman Sandifer of Byram, MS and Mamie Forman Kirk (David) of Pocahontas, TN; and her sister-in-law Cecillia Forman of Woodville, MS. She is also survived by numerous nieces and nephews.
