He was predeceased by: his parents, John Forsythe and Stella Wilson Forsythe; his wife Mary Cleta Forsythe; his daughter Diana Crocker; and his siblings, Wally Forsythe, Irene Forsythe Anson and John Forsythe.

He is survived by: his son Greg Forsythe (Penny) of Louisville; and his son-in-law Joe Crocker of Harrisville, Mississippi..
