He is survived by: his mother Mary Ann Sims (James W.); his sons, Dustin Sullivan (Michaela), Jerry Sullivan, Austin Lindsey and Justin Lindsey; and his siblings, Wayne Andrews, Mark Andrews, Autumn Murphy, Bruce, Brittany, McKenzie, Mason, Marlee Nicholson, Leslie Harding, Dawn Doba, Jerry Lynn Butler Jr. (Deloris), Shawn Butler (Amy), Timothy Butler and Nicole Butler. He is also survived by aunts, uncles, nieces, nephews, and cousins.
