He is survived by: his wife Sue Kerby; his children, Jennifer Aracri, Col. Jeff and Trish Kerby; his stepdaughter Pam Emmons (Mark Emmons); his grandchildren, Madison, Jeff Brooks, Kristen, Mitch Strange, Cameron and Kristi Kerby; and his great grandchild Carson.

The family has requested that donations be made in Jay's honor to Kindred Hospice.
