He is survived by: his wife Cheryl Crosby Sullivan; his sons, David Lamar Sullivan, Jr. (Leslie) and Michael Wayne Sullivan (Randi); his grandchildren, Kaleb William Butcher, David Lane Sullivan, Maranda LeAnn Sullivan, David Lamar "Trey" Sullivan, III and AnnMarie Adele Sullivan; his siblings, Kitty Sullivan, Joe Sullivan, Patsy Hobbs, Todd Sullivan and Brian Sullivan; and his stepmother Elouise Sullivan. He is also survived by numerous nieces, nephews, other relatives, and friends.
