She was predeceased by: her parents, Fred Streuding and Juanita Streuding; and her grandparents, B. O. and Mae Van Hook. She is survived by: her husband Rick Forte, Sr.; her children, Rick Forte, Jr. (Joy) and Angela Semanek; her grandchildren, Wade Bevis, Dr. Lauren Roberts, Angel Fagan (Matthew) and Stephen R. Semanek; her great grandchildren, Wade, Harrison, Julian and Sylvie; and her brother Phillip Streuding (Linda).
