He was predeceased by: his parents, Burley Forman and Trudy Forman; his wife Mary Agnes; his daughter Clarise Workman; his granddaughter Stacy Workman; and his siblings, Bonnelle Bohall, Nadine Arnold, Betty Rose Campbell, Bryant Foreman and Bennis Foreman.
He is survived by: his children, Dale Foreman (Melanie) of Huffman, Texas, Jody Etheridge (Tommy) of Gloster, MS and Crystal Ford (Warren) of Hull, Texas; and his siblings, Billy Foreman of Gloster, MS. and Mary Nell Borel of Meadville, MS.. He is also survived by eleven grandchildren, 19 great-grandchildren, and two great-great grandchildren.
