He was predeceased by: his wife Rhonda Suzanne Polk; and his parents, Marvin Lee Polk and Alma Coyce Stringer Polk. He is survived by: his children, Lee Anthony Polk, Jr., Jeffrey Kennedy Polk and Theresa Lynn Roberts (Michael); his grandchildren, Kayleigh Addison Polk and Dylan Avey Polk; and his sisters, Betty Lou Brown and Beverly Coyce Shultz. He is also survived by Nieces and Nephews.
