He was predeceased by: his parents, Jabez Cook Sr. and Dorothy Cook. He is survived by: his sons, John Cook (Danielle) and Tanner Cook; his grandsons, Duncan Cook, Dawson Cook and Devin Cook; and his sister Rebecca "Beckie" Ann Cook.
