He was predeceased by: his wife Laura Schleinschok McIntosh; his son Dan A. "Mac" McIntosh, IV; and his parents, Dan A. McIntosh, Jr. and Georgia Barnes McIntosh. He is survived by: his son Jason; his daughter-in-law Melissa Graham of Collins; his daughters, Pam McDaniel of Collins, Aundrea and Kyle Thames of Houston, Texas; and his grandchildren, Adam Graham, Jimmy Graham, Kadie Graham and Makenzie Thames. He is also survived by other family members and friends.
