He was predeceased by: his grandparents, Roy Moak, Jr. and Shirley Moak; and his niece Bella Rae Titus. He is survived by: his parents, Brian Wayne Moak (Tammy) and Jennifer Lee Bardwell; his wife Heather Elizabeth Moak; his children, Erika Jean Miller, Zoie LeAnn Moak and Easton Roy Moak; his grandfather Avrice Bardwell; his siblings, Harley Moak, Tamara Adams (Nathan Case), Teri Hogan (Dillon Titus), Carli Chandler (Reason), Summer Smith (Justin), Skye Tibbs (Robert), Jay Hogan (Cullen) and Caid Hogan (Allison); his sisters-in-law, Holly Fry (Jerry) and Haley Miller; his parents-in-law, John and Penny Miller; and his aunts and uncles, Melissa Adams (Steven "Stebo"), David, Jessica (Paul), Bec Bec, Jeff (Pam), Nita (Luke), Chad (Jennifer) and Chellie (Randy). He is also survived by other loving family members and friends and his faith family.
