He was predeceased by: his mother Katie Sue Givens Moak; his brother Ronald Moak; and his grandparents, Robert, Juanita Moak, Marion Clifford and Josie Nell Givens.
He is survived by: his children, Kaitlyn, Kody and Kevin Moak; his father Wayne Moak; and his sisters, Karanita Moak Brister (Dale), Malinda Moak Rippy (Jason) and Carol Moak Smith (James). He is also survived by nieces, nephews, great-nieces, great-nephews, family and friends.
