He was predeceased by: his parents, William "Bill" Forehand and Dorothy Furney Mallette; and his sister Janet Mallette. He is survived by: his children, Arthur Forehand, JaBrian Forehand, Jennifer Booth (Wes) and Melissa Forehand; his grandchildren, Kawliga, Danzey, Amos, Swayze and Hendrix; his brother Orvel Forehand (Allison); and his caregivers, Tasha and Theresa. He is also survived by numerous nieces and nephews.
