"Tony" and Gladys Creel of Back Bay; her children, Vincent Anthony Creel (Natalie) and Gerry Ann Holt (Jim); her grandchildren, Jared Holt (Ana), Jeffrey Holt (Carly), Jedediah Creel and Matthew Creel; her great grandchild Sophia (Baby Holt); her siblings, Dr. Matthew F. Kuluz, Vincent Kuluz (Jean), Pat Kuluz (Linda) and Donna Watts; and her caregivers, Ellen Williams, Natasha, Deidre and Teresa of KareMed.
