He was predeceased by: his parents, Bernard A. "Lefty" Blaize and Mildred Blaize. He is survived by: his children, Shelley Cuevas (Gene), Julie Sloan (Laura), Vicki Blaize (Misty), Don A. Blaize Jr. (Thad), Andrew Blaize (Sarah), Stacey Blaize and Ernest James Falgo (Amanda); and his grandchildren, Alexandra, Brittany, Elizabeth, Nicholas, David, Grace, Rosie, Mary, Harley, Zoya, Jay, Alderik, Kiefer and Amara. He is also survived by eleven great-grandchildren.
