He is survived by: his wife Carolyn Louise Addkison of Joseph's High School; his siblings, Ditter Ingle of Rockport, TX, Theresa Johnson of Laguna Niguel, CA and Mark Cook of Florence, MS.; his sons, Stephen Cook of Stone Mountain, GA and David Cook (Jennifer) of Manitou Beach, MI; his grandchildren, Julia Dean of Glascow, Gage Dean of Rossvile, GA and Savannah Burkhart (Devin) of Ringgold, GA; and his great grandchild Ava Jade Burkhart of Ringgold.
In lieu of flowers the family requests donations to one of Willis' favorite charities, Mercy Ships, or to St. Mary's Hospice here in Georgia.
