He was predeceased by: his wife Judy C. Stevens (Mask). He is survived by: his children, Rhonda Karlsson (Stevens) and Jerry Dale Stevens II; and his sister Janet Earnest (Gary). He is also survived by five grandchildren, four great-grandchildren.
