He is survived by: his son Weston Michael Lee Woolfolk; his fiancée Janelle Pulliam; his mother Stephanie Lynn Woolfolk (Jason Beach); his siblings, Crystal Lynn Woolfolk, Michael Francis Woolfolk, III, Katherine Kay Woolfolk of Richmond, Missouri and William Lawrence Woolfolk of Richmond, Missouri; his grandfather Michael Black of Dallas, Texas; his aunts, Stacy Black of Midland, Mary Glore of Raytown and Michelle Clark of Blue Springs; and also, Gary and April Pulliam. He is also survived by cousins, dear friends.
The family suggests memorial contributions be made to the Stephen Michael Woolfolk Memorial Fund.
