He was predeceased by: his parents, David Nathan and Edna Marilyn McIntosh Phillips, Sr.; his grandson Joseph Wallace; and his siblings, Donna Nelson, Shirley English, Mary Jane Page, Robin Phillips, Vickie Lair, Wayne "Jake" Murphy, Sr., Oral "Butch" Murphy and Rocky J. "Brov" Phillips.
He is survived by: his wife Lora Phillips of Williamstown; his children, David Nathan Phillips, III (Mandy Mesecher), Danika Wallace (Wally) of Ewing, Carlie Phillips of Fairview and Nate Phillips (Caitlin Reutzel); his grandchildren, Memphis Phillips and Kashton Wallace; and his siblings, Marilyn Garrells, Linda Shirey (Dan), Angela Schaller (Rock), Yvonne Dudgeon (Larry), Carmon Lindsey, Pam Briscoe (Randall), Steve Phillips (Stephanie) and Gretchen Phillips. He is also survived by several nieces & nephews.
