He was predeceased by: his parents, Donald Lewis and Lois Tyree Lewis; and his siblings, Beverly Noble and Ken Lewis. He is survived by: his wife Opal Earlene Lewis; his daughters, Jeannie Lewis, Evette Prill and Erica McMillian (Tatum); his grandchildren, Ashley Painter (Jared) of West Plains, Nicole Bennett of West Plains, Megan Cale of West Plains and Logan McMillian of West Plains; and his siblings, Danny Lewis (Linda), Lanny Lewis (Dee) and Sherry Martin (Bill). He is also survived by several nieces and nephews.
