He is survived by: his wife Deborah Kotek; his mother Jean Kotek; his sons, Ryan Kotek (Laura) of Jacksonville, FL and Justin Kotek (Kimberly); his stepdaughter Danette Eubank (Shane) of Pekin, IL; his siblings, David Kotek of Milwaukee, WI, Daniel Kotek of Milwaukee, WI, Karen Thomas of East Peoria, IL, Jane Young (Thomas Dechnik) of Villa Park, IL and Connie Wexler of Lawrence, IN; his niece Tammy; his nephew Phillip; his granddaughters, Adeline, Audrey and Cora; his stepgrandchildren, Ryan, Jonathan (McKinsey) and Alexes; and his stepgreat-grandchildren, Kenway and Cooper.
