He was predeceased by: his parents, Carl Leslie and Juanita Hook (Rex); and his siblings, Jo Ann Shackelford (Hook) and Gerald Hook. He is survived by: his wife Joyce Hook; his children, Robin Luber (Kevin), Kelly Calhoon (Clint Springer) and Todd Hook (Sue); his grandchildren, Rachel McKenzie (Larry), Joshua Luber, Cody Mayberry (Tiffanie), Dakota Hook, Hunter Calhoon, Andy Chaffin (Kelli) and Brent Chaffin (Mindy); his sisters, Martha Yancey, Leslie Adams and Cheryl Hook; and his siblings-in-law, LaDean Kirchhoff (Lee), Gale Allen and Gary Allen (Dolores). He is also survived by 12 great grandchildren.
