He is survived by: his wife Angela Dempsey; his children, Zaid Forkohl of Dexter, Dayton Forkohl of Dexter and Breyton Slaughter of Puxico, Missouri.; his parents, Charles Howell of Neelyville and Cynde "Bean" Shepherd of Poplar Bluff; his grandmother Carolyn Shepherd of Poplar Bluff; and his siblings, Jacquilyn Daniels of Williamsville, Janie Howell of Williamsville, Alexus Jyden Shepherd of Poplar Bluff, Colton Howell of Poplar Bluff and Austin Shepherd of Poplar Bluff.
