He was predeceased by: his parents, Freeman Middleton and Alpha Middleton (Anderson); and his brother Sherman Lewis. He is survived by: his wife Sharon Middleton; his children, Timothy Middleton (Joy) of Piedmont and Kimberly Howe (David) of Knob Noster; his grandchildren, Nicholas Middleton (Selena), Sydnee Ivy and Sam Ivy; his great grandchildren, Bash and Colby; and his brother Stanley Lewis of Geary, OK.. He is also survived by his family and friends.
